Symptoms of High Estrogen on TRT
How to spot the signs of this common side effect and get your hormones back in sync.

Too much estrogen might seem like the last thing you need to worry about if you’re a man. And it may seem even more unlikely if you’re on testosterone replacement therapy (TRT), a treatment to increase levels of the OG male hormone.
But developing high estrogen on TRT is fairly common—and easily addressed. And while men need a moderate amount of estrogen to help regulate fertility, libido, cognition, bone and muscle mass, and metabolic health, too much can lead to a hormone imbalance and trigger unwanted side effects.1 2
When TRT drives estrogen levels above the normal range, you might notice breast tenderness, gynecomastia (breast tissue growth), mood swings, increased belly fat, erectile dysfunction, or reduced sex drive. Left unchecked, high estrogen levels in men may also slightly increase long-term risks for heart disease, breast and prostate cancer, and type 2 diabetes. 3

How TRT Can Cause High Estrogen
In one study of injectable testosterone, about 15 to 20 percent of men developed elevated estrogen levels—a condition called hyperestrogenism.4
High estrogen on TRT is usually related to aromatase—the enzyme that transforms testosterone into estradiol (E2), the most potent form of estrogen. Men’s bodies use estrogen to help regulate libido, erectile function, and sperm production, and build bone strength.
Here’s why you might develop high estrogen:
- When you increase testosterone levels through therapy, there’s more testosterone for aromatase to convert to estrogen.
- Smaller amounts of estrogen are also made in fat tissue, skin, bone, and even the brain—which means the more body fat you have, the more aromatase you have converting testosterone into estrogen. 5 6
- Aging adds to the problem—older men typically have more fat and less muscle, further ramping up aromatase activity. 7
- Dosage matters: The higher your TRT dose, the more testosterone your body has available to turn into estrogen.
“In men, about 90 percent of the estrogen we have floating around comes from converting testosterone into estrogen wherever it’s needed—for building bone and maintaining libido—through the enzyme aromatase,” says endocrinologist Steven Wise, M.D.
Normal estradiol levels in adult men range from 10 to 40 pg/mL, though this can vary slightly by lab. Levels above 40 pg/mL are often labeled “elevated,” but not every elevation is clinically significant. Most physicians treat based on both symptoms and lab trends—typically intervening only when estradiol stays above 50–60 pg/mL and causes noticeable side effects.
High estrogen levels can affect testosterone
If estrogen levels climb too high on TRT, it can set off a vicious cycle of hormone imbalance. Elevated estrogen slows communication along the hypothalamic-pituitary-gonadal (HPG) axis—the system that tells your testes to produce testosterone. As testosterone drops, estrogen becomes even more dominant, which further suppresses testosterone, and the cycle continues.
Starting TRT with elevated estrogen levels—due to age, excess body fat, or metabolic conditions like diabetes or liver disease—can make this imbalance even easier to trigger if hormone levels aren’t closely monitored.
High estrogen in bodybuilders
Bodybuilders who take synthetic anabolic testosterone for muscle gains often have high estrogen levels because the dose of testosterone is 10 to 100 times higher than what’s prescribed for TRT. 8 As mentioned above, the higher your testosterone level, the greater the conversion into estradiol—and the higher the risk for estrogen-related side effects like gynecomastia, bloating, and mood swings.
“The mechanism of testosterone being converted to E2 is amplified,” says urologist Joshua Calvert, M.D. In other words, when someone takes doses higher than what the body naturally makes, it overwhelms the enzyme that converts testosterone to estrogen, causing estrogen levels to spike.
Unlike medical TRT, which restores testosterone to healthy, age-appropriate levels under physician supervision, anabolic steroid cycles push hormones far beyond the normal range. Many bodybuilders also “stack” steroids with other compounds such as human growth hormone (HGH), insulin, and IGF-1, all of which affect testosterone and estradiol differently and can further disrupt hormonal regulation.
Symptoms of High Estrogen on TRT
Symptoms of high estrogen can be physical, sexual, or emotional, and often overlap with symptoms of low testosterone. Lab testing is the only way to know for sure whether low testosterone or high estrogen levels are causing your symptoms.

Gynecomastia
One of the most visible signs of high estrogen levels on TRT is gynecomastia, an enlargement of male breast tissue (man boobs). Estrogen can stimulate the same receptors in male breast tissue that drive breast development in women, which can lead to swelling, tenderness, and nipple sensitivity. 9 10
Recent literature and expert reviews continue to describe gynecomastia on TRT as possible but “uncommon and often transient”—meaning that if it happens it typically doesn’t last—when taken under medically supervised, monitored regimens. 11
Water retention
Mild puffiness in the hands, ankles, or face is a common early side effect of TRT, 12 since rising estradiol levels alter the body’s balance of water and sodium. It might seem like fat gain—but it’s actually just fluid.
Mood swings
High estrogen levels can trigger irritability, moodiness, and even depression. Research in men being treated with TRT for low testosterone has shown an association between above-normal estrogen levels and serotonin receptors in the brain that impact mood. 13 14 And a 2025 cross-sectional study found greater rates of depression and anxiety among men taking anabolic steroids who had elevated estradiol levels. 15
Erectile dysfunction
High estrogen can lead to erectile dysfunction (ED) and related problems, including low libido and reduced fertility. Estrogen receptors throughout the penis and testes help regulate nitric oxide—the molecule that enables erections by increasing blood flow to the penis—but too much estrogen can blunt that response.16 17 In a 2020 study, men with high estradiol were significantly more likely to experience erectile dysfunction and other sexual issues, including premature or delayed ejaculation.
Weight gain
High estrogen from taking TRT often shows up as excess fat around your middle. A 2015 study found that men with the highest estradiol levels on TRT had the least muscle mass and the most abdominal fat. Men on TRT often gain muscle, but if more testosterone is converted to estradiol, then there’s less free testosterone available to build lean mass. So as estrogen rises, muscle tone drops—a frustrating reversal of what TRT is meant to do.18
Men who start TRT overweight also might be more likely to develop high estrogen levels. Because aromatization happens in fat cells, the more body fat you have, the more estrogen your body produces—creating a loop that makes fat loss even harder.19
Trouble gaining muscle
While the correct dose of testosterone can help improve body composition, an imbalance of testosterone to estrogen can actually lead to muscle loss—because there’s less free testosterone available to build muscle mass.
A 2015 study found that men on TRT with the highest estradiol levels had the lowest total lean muscle mass, particularly in their abdomen and upper body. And there was a dose response, in that as E2 went up, fat mass went up and lean muscle went down. 20 “When you have excess estrogen, you can work out and work out, and you just can never seem to make any muscle definition,” says Wise.
Risks of High Estrogen
If high estrogen on TRT is unaddressed, it could increase your odds for a number of conditions that go beyond water retention, breast tissue growth, and moodiness. High levels of estrogen have been linked to an increased risk for:
- insulin resistance 21
- type 2 diabetes 22
- blood clots 23 24
- high blood pressure 25
- prostate cancer 26
- breast cancer 27
The research behind some of these risks is associated more strongly with women, Calvert says, but hormone imbalance will affect men’s overall health. TRT use increases aromatization—driving up estrogen levels that stimulate receptors in breast and prostate tissues and possibly making you more prone to these cancers. 28
The science on all of these risks is still emerging, and more is needed to show a clear cause and effect between high estrogen in men and chronic diseases.
Anastrozole for High Estrogen on TRT
A common way of managing high estrogen levels on TRT is taking anastrozole. As an aromatase inhibitor, anastrozole is a prescription medication that blocks the enzyme aromatase from converting testosterone into estrogen. Around 30 percent of doctors prescribe an aromatase inhibitor like anastrozole to men who are showing symptoms of high estrogen on TRT. 29
“If a patient is symptomatic—they have breast tenderness or irritability—sometimes just going down on the TRT is enough to make estrogen levels normal again,” says Wise. “Even then, I might put them on an aromatase inhibitor, just to be sure to get the estrogen down, and then back it off later if their levels get too low.”
How to avoid high estrogen while on TRT
The best way to avoid developing high estrogen on TRT is to work with a doctor who monitors your dose and testosterone and estrogen levels while you’re on treatment.
Lab tests are recommended about every three months for the first year to monitor your levels of both testosterone and estrogen, and then less often after that if you’re not experiencing adverse side effects.
The Bottom Line
TRT can raise estrogen levels, causing symptoms like breast tenderness, gynecomastia, mood swings, and erectile dysfunction, along with a possible increased risk of heart disease and certain cancers. Regular blood tests can ensure levels stay in check, and if estrogen runs high, an aromatase inhibitor such as anastrozole can help restore balance.
