What is Estradiol Gel Prescribed For?
Estradiol gel is a prescription form of systemic estrogen therapy used to treat menopause symptoms caused by changing estrogen levels, especially hot flashes and night sweats. It may be prescribed as part of menopausal hormone therapy (HRT) during perimenopause or menopause when systemic estrogen is appropriate.
Estradiol gel contains 17β-estradiol, a bioidentical form of estrogen that is structurally identical to the estradiol produced by the body. The gel is absorbed through the skin and into the bloodstream, providing a transdermal alternative to estrogen pills and patches. For women who dislike taking a daily pill, have trouble keeping patches in place, or are sensitive to patch adhesives, estrogen gel can offer another convenient way to use systemic HRT.
How Does Estrogen Gel Work?
Estradiol gel delivers 17β-estradiol through the skin and into the bloodstream, helping replace estrogen as levels fluctuate and decline during the menopause transition. Systemic estrogen is the most effective treatment for menopause-related hot flashes and night sweats and can also help prevent bone loss associated with menopause.
Because estrogen gel is absorbed through the skin, it bypasses the liver’s first-pass metabolism. Compared with oral estrogen, transdermal estrogen has less effect on triglycerides and certain clotting factors, although all systemic estrogen therapies have potential risks that should be reviewed with a physician.
Who is Estrogen Gel Right For?
Estradiol gel therapy is designed for women who need systemic estrogen support. It may be appropriate for women who:
- Are experiencing symptoms of perimenopause or menopause (e.g., hot flashes, night sweats, mood changes)
- Have low estradiol levels confirmed by labs
- Are transitioning from estradiol patches due to poor adhesion or supply shortages
- Have sensitivity or allergic reactions to patch adhesives
- Prefer a daily, adjustable transdermal option over patches or oral therapy
Estradiol gel provides an effective, convenient option for systemic menopause HRT without the need to swallow a daily estrogen pill or wear a patch. A Hone physician can review your symptoms, medical history, and health risks to determine whether estradiol gel is an appropriate option for you.

Warnings
Talk with your physician about your medical history and any medications or supplements you take before starting estradiol gel. Systemic estrogen requires appropriate medical monitoring and is not right for everyone. In women who have a uterus, using estrogen without adequate endometrial protection increases the risk of endometrial cancer; a physician will determine whether progesterone or another progestogen should be part of the treatment plan.
